These are condition cards for Cubicle 7′s Warhammer Fantasy Roleplay 4th Edition.
They are brought to you by Herr Prof. Frettchen. His “Portable Bills of Old World Conditions” can be used as a reference during gameplay to track the the multitude of horrible things that can be inflicted on one’s character.
There are two orientations, one tall and narrow the other more square. The second page provides a backing image if you can print them double-sided.

Update 9/4/2018: There has been some demand for versions without the background ink splatter (What did you think those drops were? Printing is messy!). Here are some “clean” versions that are less grim but equally, if not more, perilous.

I would *love* for you to make small condition cards with condensed info (treating the small space available as an opportunity to boil down the informational text to only convey critical info).
European mini sleeves would be perfect. The dimensions are: 1 11/16″ x 2 5/8″ (44×68 mm)
